Since the beginning of the epidemic, HIV mother-to-child transmission has been recognised as a major public health issue. Despite the great efforts done to reduce HIV infection among children, in 2015, more than 150,000 children became infected with HIV. Early combination antiretroviral therapy (ART) initiation in vertically infected infants with HIV has had a major impact on clinical outcomes reducing morbidity and mortality. Moreover, early initiation of ART leads to a significantly smaller reservoir size and a better immune reconstitution. However, definitions of early vary a great deal and literature reporting on ART initiation in the first year of life rarely reports on outcomes when treatment is initiated in the neonatal period.
