The aim of this case report is to describe the abnormalities of tooth shape and particularly the dens evaginatus. For this purpose a case of a twelve years patient visiting the Policlinic of the European University of Madrid (UEM) with four dens evaginatus localized in first and second premolars with immature apexes is presented. After a proper clinical and radiological diagnostic we proceeded grinding the tubercles in five visits during one month in order to avoid occlusal and pulp complications. Seven months after the procedure, the patient did not present any symptomatology associated and the correct root formation can be ascertained in the affected teeth. Despite the low prevalence, dens evaginatus is an abnormality requiring an early and comprehensive diagnostic and a proper treatment in order to avoid the complications associated.
